Role Description

This is a full-time on-site role for an RN - Care Coordination Lung Navigator located in Wausau, WI. The RN will provide comprehensive care coordination for patients with lung conditions, overseeing patient assessments, care plans, and patient education. The role involves collaborating with multidisciplinary teams to ensure continuity of care, coordinating diagnostic and therapeutic procedures, and offering emotional support to patients and their families. The RN will also monitor patient progress and adjust care plans as needed to achieve optimal health outcomes.

Qualifications

• Registered Nurse (RN) license in the state of Wisconsin or eligible for licensure
• Experience in care coordination, patient education, and case management
• Strong clinical skills and knowledge of lung disease management
• Excellent communication, interpersonal, and organizational skills
• Ability to work collaboratively with a multidisciplinary team
• Bachelor’s degree in Nursing or related field preferred
• Basic Life Support (BLS) certification
