A company is looking for a Care Coordinator to support chronic care management by coordinating non-clinical care activities and providing administrative support.
Key Responsibilities
Coordinate non-clinical components of patient care plans, including referrals and appointment scheduling
Act as a primary resource for patients and families for routine questions and escalate clinical issues as needed
Facilitate the organization and reconciliation of external medical records and maintain accurate documentation within the electronic health record (EHR)
Required Qualifications
Minimum of a High School diploma
Completion of a Medical Terminology course preferred
Medical office experience preferred
Five years of work experience preferred
Ability to pass a comprehensive background check upon hire and throughout employment
