Pinnacle Home Care, Florida's largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we're looking for a Referral Outcomes Coordinator to join our award-winning team.

Key Responsibilities :

• Accurately process non-admits in accordance with company policies and compliance requirements.
• Document all non-admit decisions and rationale in the appropriate systems (e.g., Allscripts, Kantime, internal trackers).
• Collaborate with clinical and operational teams to ensure all documentation is complete before case closure.
• Track and monitor non-admits to identify trends and escalate concerns to leadership.
• Maintain organized, up-to-date records for auditing and reporting purposes.
• Provide timely updates to Regional Directors, Referral Development Specialists (RDS), and other stakeholders.
• Support reporting processes by contributing to daily, weekly, and monthly non-admit metrics.
• Participate in team meetings and contribute to process improvement initiatives.
