Job descriptionProvide individual and group counseling with a focus on substance use recovery and harm reduction Complete all assigned community-based visits within a 7-hour workday Transport clients when necessary (e.g., hospital discharges) Facilitate group sessions aligned with each client's treatment plan Complete initial assessments, treatment plans, and all required documentation within established timeframes Submit progress notes on the day of service and sign within 48 hours Complete discharge documentation and 30, 60, and 90-day follow-ups Coordinate with medical staff to obtain hospital discharge paperwork and update client files Report significant events and incidents per policy Perform other duties, as needed Master's Degree in Social Work, Mental Health Counseling, Psychology, or Rehabilitation Counseling CASACs without a master's may be considered based on experience Solid assessment and documentation skills Minimum 1 year of experience in a Clinical or Rehabilitation setting
A community-based outpatient mental health program is seeking a Substance Abuse Specialist to join its Personalized Recovery-Oriented Services (PROS) team. This is a clinical role providing recovery-focused, harm-reduction services to individuals with co-occurring mental health and substance use conditions.
