About The Position

Job Title : Registered Nurse

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience depending on the specialty

Certifications : CPR, BLS

Must-Have : Proficient in HomeCare HomeBase and Oasis EMR systems, recent specialty experience

Description :

Provides coordinated and directed home nursing care according to physicians' orders.

Care is based on agency policies and procedures through the competent application of the nursing process.

Responsible to the Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.

Home visits and periodic office meetings are required.

Must have reliable transportation and current driver's license.

Agency-required liability insurance is also necessary.

Responsible for an average of 5-10 patients per day.

Each week, 30 productivity points are expected.

Different types of visits and activities hold varying point values.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Expectation to accept every patient on the schedule, regardless of distance.

All documentation must be completed within 24 hours of patient appointments.

Understanding that there are no guaranteed hours with this contract is crucial.

Must be willing to travel up to 75 miles from the assigned location.

Mileage is reimbursed at the IRS Standard rate.

All travelers must be proficient with HomeCare HomeBase and Oasis systems within the last 6 months.

Local candidates may be accepted.

No pending licenses will be considered; the license must be held.

Experience in hospice care is required.

Weekly productivity expectations should be met to ensure job satisfaction.

A commitment to patient care and documentation accuracy is essential.
