Registered Nurse (RN)

A company is looking for a Registered Nurse (Full Time) to provide patient-centered health coaching and chronic disease management.

Key Responsibilities

Deliver patient-centric health coaching integrating medical needs with patient preferences

Triage patient-reported symptoms and coordinate care with providers and support teams

Document patient care accurately and consistently in a telehealth setting

Required Qualifications

Bachelor of Science in Nursing (BSN) required

Active Registered Nurse license in good standing

Certified Diabetes Care and Education Specialist (CDCES) or willingness to pursue certification

3+ years of direct patient care in an ambulatory or telehealth setting

Proficiency with digital tools for care delivery
