Patient Intake Coordinator
Key Responsibilities :
Serve as the first point of contact for new patient referrals.
Communicate with patients, caregivers, and insurance representatives to complete the intake process.
Verify eligibility, collect required documentation, and ensure all pre-enrollment steps are completed accurately.
Coordinate with nursing, compliance, and staffing departments to ensure a smooth start of care.
Maintain detailed records in internal systems and ensure compliance with all DOH and agency guidelines.
Patient Retention Coordinator
Key Responsibilities :
Build and maintain positive relationships with existing patients and caregivers to promote satisfaction and continuity of care.
Conduct follow-up calls to ensure patient needs are met and address any service-related concerns promptly.
Collaborate with nursing and scheduling teams to minimize service interruptions.
Identify and report patterns that may affect patient retention or satisfaction.
Participate in retention initiatives and campaigns to improve patient engagement and loyalty.
