Who We're Seeking : Are you a dedicated and compassionate Medical Social Worker (MSW) looking for a fulfilling opportunity to support home health patients? We are currently seeking a dynamic individual to join our collaborative Home Health team, where you'll have the opportunity to provide direct support to individuals in the comfort of their homes.
Your Responsibilities : As a member of our dedicated team, you'll :
- Develop trusting, meaningful relationships with your clients over extended periods of time – something that is not possible in a hospital or facility setting.
- Develop, communicate, and coordinate a goal-oriented plan of care in collaboration with the patient, their medical doctor, and family. This includes care plans for LNAs as needed.
- Travel in a designated territory within Washington County or an adjacent town.
- Maintain consistent, timely, and accurate communication with all members of the patient’s health care team.
- Keep up to date electronic documentation of care provided.
- Evaluate each person’s progress in meeting their goals of care and update plan of care accordingly.
- Evaluate evidence-based treatments for clients with psychosocial challenges that interfere in their ability to abide by medical plan of care.

Master’s or doctoral degree from a school of social work accredited by the Council on Social Work EducationMinimum one year of progressive clinical experience in medically-oriented setting.Exceptional new graduates will be consideredAbility to travel to clients within a designated territoryReady to Make a Difference?
