Job descriptionThe RN Case Manager follows patients, plans discharges, and collaborates with multidisciplinary team members. Works in coordination with Utilization Review, with knowledge but no direct responsibility for it. Functions as part of a small case management team with potential assignments on various inpatient units, including Med / Surg, ICU, Telemetry, and Stepdown. Assists with Special Care Nursery and PACU needs; no behavioral health assignments. Ensures medical necessity of hospitalization and treatment per hospital and payer guidelines. Ensures appropriate utilization of hospital resources and collaborates on cost-effective and efficient care management. Responsible for integration, coordination, and advocacy for patients and families requiring post-hospital services. Achieves planned care outcomes by planning and arranging discharge services. Acts as a liaison with medical staff, other departments, patients, families, and community resources to facilitate utilization management and discharge planning processes. Familiarity with charting systems such as EPIC.

- Familiarity with Medicare, Medicaid, managed care, and other third-party payer review requirements
