Physical Therapist (PT) – Home Health

Location : York, PA

MedAdventures is seeking an experienced and compassionate Physical Therapist (PT) to join a leading Home Health team in York, PA . This position offers the chance to provide one-on-one care to patients in their homes—helping them regain strength, mobility, and independence in a setting that’s both comfortable and personal.

We’re looking for a clinician who is confident working independently in the field and experienced in documentation systems used in home health care.

    • Key Responsibilities :

  • Perform comprehensive in-home physical therapy evaluations and create individualized care plans
  • Deliver hands-on therapy to help patients achieve strength, mobility, and balance goals
  • Educate patients and caregivers on home exercises and fall prevention strategies
  • Accurately document visits and progress using OASIS and Homecare Homebase (HCHB)
  • Collaborate with nurses, therapists, and case managers to ensure coordinated, high-quality care

    • Qualifications :

  • Active Pennsylvania Physical Therapist license (or eligibility to obtain)
  • Experience with OASIS and Homecare Homebase (HCHB) required
  • Prior home health experience preferred
  • Excellent communication, time management, and clinical reasoning skills
  • A passion for delivering patient-centered care in a home environment
