  • Pay : $35.13 - $52.70 / hour

Summary

  • This position Provides patient / family education.Participates in family conferences. Responsible for all necessary documentationas required by accepted medical guidelines. This position (Outpatient / Inpatient) works under the supervision ofDirector. Evaluates, plans and implements treatment programs for therehabilitation of patients, as directed by a provider, using approved treatmentprocedures.
  • This position works under the supervision of the Director and is employed by Mosaic Health System.

    • Duties

  • Evaluates patients referred by providers.
  • Establishes goals that maximize the patient's recovery.
  • Plans a treatment program, using appropriate treatment procedures that will best accomplish the established goals.
  • Implements the treatment program using approved procedures.
  • Documents treatment procedures and the patient's response to them, on a regular basis.
  • Other duties as assigned

    • Qualifications

  • All required education is a minimum requirement. Higherlevels of education are acceptable. Bachelor's Degree- Graduate of accreditedschool of physical therapy required.
  • Licensed Physical Therapist in State of Missouri required.Heart Saver certification or BLS required.
