Job descriptionSick Leave - 13 days Paid Holidays - 13 annually Administrative Time Off - Up to 10 days Volunteer Time - 16 hours annually to Volunteer Retirement - 401(k) with 4% Employer Match Employer-Paid Life Insurance, AD&D, and Long-Term Disability Employee Assistance Program (EAP) College Tuition Discount - Up to 20% off Weekly Direct Deposit Schedule : Monday-Friday (8am - 5pm) Caseload : 8-13 Patients per Day Call : 1 in 6 or 7 Weeks Provide quality care through implementation and evaluation of individualized patient care plans, discharge planning and complex outpatient needs. Perform prospective, concurrent and retrospective evaluation of clinical information to establish medical necessity, appropriateness of services and cost effectiveness. Serve as patient and family advocate during hospital stay and outpatient status. Conduct in-home or hospital evaluations in some circumstances. Associate's Degree in Nursing Registered Nurse License in the State of Florida Certification in Case Management (or the ability to obtain within one year of employment) Minimum of 2 years of experience as a Medical Case Manager Current BLS certification

- The salary range is $75,000 to $95,000 per year
