Job Description : We are seeking a skilled and dedicated Home Health Physical Therapist to join our team. The ideal candidate will be experienced in providing comprehensive physical therapy services to patients in their homes, have experience in lymphedema management and performing PT start of cares. The candidate should also possess proficiency in conducting OASIS assessments, ensuring accurate and thorough documentation of patient progress.

Responsibilities :

Conduct initial patient assessments, including comprehensive evaluations and OASIS assessments.

Develop personalized care plans based on patient needs, medical history, and physician orders.

Provide lymphedema management and treatment, leveraging your specialized certification in lymphedema.

Guide patients through therapeutic exercises and activities to enhance mobility and functionality.

Collaborate closely with the interdisciplinary team, including physicians, nurses, and other therapists, to ensure cohesive patient-centered care.

Monitor and track patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.

Maintain accurate and timely documentation of patient assessments, interventions, and progress.

Educate patients and their families on home exercise programs and self-care techniques.

Adhere to industry best practices, ethical standards, and relevant regulations.
