• Nurse Navigator LPN or RN Comprehensive Disease Management (Palliative Care)
• Full Time; Monday through Friday 8am - 5pm
• Wichita, KS and surrounding areas

The Comprehensive Disease Management (CDM) / Palliative Nurse Navigator helps patients navigate their healthcare journey by having a goal of care conversations, identifying their healthcare needs, recommending resources, communicating with patient's healthcare providers and educating on disease progression and treatment / care options available. In an overwhelming time where patients and families need to make critical healthcare decisions, CDM Nurse Navigators are able to walk this journey with people so they can confidently make healthcare decisions that align with their personal goals.
