Job DescriptionOverview
The Licensed Vocational Nurse (LVN) reports to the Nurse Manager and is responsible for managing the individualized patient care by promoting and restoring patients' health through the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients.
Essential Functions and Responsibilities

Provides interdependent functions that overlap the practice of medicine that require standardized procedures; follows appropriate guidelines in accordance with the organization’s written policies.Processes CMR reports to county for reportable (+) STD’s.Assists with Prior Authorizations.Reviews patient labs.Triages patient phone calls, portal, and voicemail messages.Administers medications / draws blood samples / starts IV access (if certified) as appropriate.Supervises workflow of Medical Assistants under the direction of the Nurse Manager.Ensures clinic runs efficiently and on time.Assists with patient outreach for PrEP and PEP patients; ensures that patients follow up with Prevention Coordination care team and that future appointments are placed.Operates within scope of practice per CA Nurse Practice Act.Demonstrates an in-depth knowledge of medical and clinical procedures and terminology.Performs appropriate assessments and treatments per nursing care plan, or as ordered by medical provider, in an accurate and timely manner.Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following clinic and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.Resolves patient problems and meets needs by utilizing multidisciplinary team strategies.Maintains safe and clean working environment by complying with procedures, rules, and regulations; calls for assistance from health care support personnel.Protects patients and employees by adhering to infection-control policies and protocols, proper medication administration, storage and disposal procedures.Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.Reviews lab values; performs medication reconciliation; reports abnormal findings to provider and intervenes as appropriate.Documents patient care services by charting in patient and department records.Maintains continuity among nursing teams by documenting and communicating actions, irregularities and continuing needs.Maintains patient confidence and protects operation by keeping patient information confidential.Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s diversity, preserving their autonomy, dignity and rights.Attends and participates in weekly / monthly staff meetings, case conferences, in-services and other meetings as assigned.Monitors compliance with OSHA, HIPPA and all other applicable regulationsMaintains professional and technical knowledge by attending educational workshops; reviewing professional publications, participating in professional societies, and renewing certifications.Other duties may be assigned, as required.Qualifications
Must possess a valid license as a nurse (LVN) in the state of California.Current Basic Life Support certification.Must be able to show proof of Flu COVID-19 vaccination on first day of hire.Minimum of two years’ experience in direct patient care as an LVN Nurse.Experience in the HIV / AIDS, Hepatitis C and chronic diseases care / management preferred.Basic knowledge of other infectious diseases and state / federal reporting requirements.Demonstrate knowledge of effects of psychosocial needs, trauma history, and cognitive / behavioral / motivational functioning on health-related behavior and exhibit ability to intervene appropriately and effectively.Have the appropriate level of behavioral maturity and patience in handling a variety of patient types and needs.Have diplomacy and tact in handling difficult and irate patients.Ability to work effectively with men and transgender women of diverse races, ethnicities, ages, and sexual orientation in a multicultural environment.Any additional certification will be considered an advantage.Company Requirements
Must be able to pass a pre-employment drug test, physical, and a background check to include a 7-year criminal, 10-year SSN & employer history reference check.Excellent interpersonal skills.Attention to detail.Must be able to work flexible schedules.Must take yearly flu shot or wear flu mask during flu season for patient-facing positions and test for tuberculosis as required by the Centers for Disease Control and Prevention.
