Key Responsibilities :

Assess patient needs and develop individualized care plans.

Administer medications and perform skilled nursing procedures in the home setting.

Provide education and support to patients and families regarding disease management, recovery, and end-of-life care.

Collaborate with interdisciplinary healthcare teams to coordinate patient care.

Monitor patient conditions, document assessments, and maintain accurate clinical records.

Deliver compassionate end-of-life support for terminally ill patients, focusing on comfort and dignity.

Ensure patient safety and comfort while providing holistic and comprehensive care.

Qualifications :

Current Montana RN license (required).

Minimum of 1–2 years of nursing experience in both Home Health and Hospice (preferred).

Current BLS certification.

Valid driver’s license with a clean driving record.

Strong clinical assessment and critical thinking skills.

Compassionate, patient-focused, and able to provide support to families during challenging times.
