Job descriptionDocumentation System / EMR : Plexus Average Number of Surgical Cases Per Day : 25 Trauma Level / Designation : 3 Annual ER Volume : 23555 Description of OR Staff : Four part-time MDA's. three full-time CRNA's Number of Operating Rooms : 5 Certification Requirement (BLS, ACLS, PALS, etc.) : all 3- BLS, ACLS, PALS Patient Age Demographic (Pediatric, Adolescent, Adult, Geriatric) : All Ages Will call be in house? No Additional cases : Ortho, general surgery, scopes, dental, uro AANA Board Certified - Required BLS, ACLS, & PALS - Required MI license at time of submission - Required Experienced CRNA with regional block and OB experience - Required Providers MUST participate in MI Medicaid / CHAMPS - required at time of presentation Clean background for quick credentialing - Required Please list availability at time of name clear COVID vaccination - Required at time of name clear
