SUMMARY :

This position will provide case management services to residents of Supportive Housing. The Case Manager provides direct services to residents including the completion of needs assessments, care plans, periodic reassessments and overall comprehensive service coordination. This position reports to the Program Director.

Essential Duties and Responsibilities :

Core duties and responsibilities include the following. Other duties may be assigned.

  • Provide holistic and comprehensive case management services.
  • Provide on-site crisis intervention as needed.
  • Facilitate the intake and assessment process and develop care plan.
  • Educates participants with respect to good housekeeping and home-safety practices, budgeting and landlord / tenant relations, as necessary.
  • Maintains appropriate documentation of all tenant contacts.
  • Documents monthly progress notes in the participants’ chart.
  • Facilitate on-site support groups and recreational activities.
  • Conducts home visits to help evaluate participants’ compliance with the rules and regulations of the program and to assess the need for referral to supportive services.
  • Acts as liaison between participants and building managers to ensure all repairs are completed to either the participant’s apartment unit or the building.
  • Maintain knowledge of community resources and counseling / social work practices with high risk populations.
  • Connect participants with needed and available community resources.
  • Assess eligibility and assist with benefits and entitlements.

  • QUALIFICATIONS :

    EDUCATION AND / OR EXPERIENCE :

  • Master’s degree in social services or related clinical field and 2 years’ experience working with applicable populations; or
  • BS / BA Degree in a human services related discipline and 5 years’ experience working with applicable populations.
  • Experience working with homeless populations and those with SMI and / or SUD
  • OTHER SKILLS AND ABILITIES :

  • Must have strong interpersonal skills to support leadership, management, negotiation and problem-solving functions of this role.
  • Excellent judgment and discretion; ability to handle multiple priorities simultaneously, meet deadlines, and handle work-related stress is required.
  • Friendly, courteous, service-oriented, professional, outgoing, and customer service oriented.

  • Tuesday - Saturday 10AM - 6PM; 11AM - 7PM on Thursday's.
