License : Valid state license required.

Certifications : Must provide resume and license verification.

Must-Have :

• Completed prescreen form
• Three references
• Criminal background checks (county, federal, multi-state, sex offender)
• Social Security number trace
• PPD or Quantiferon test results

Description :

Ensure the prescreen form is filled out and attached.

Resume and license verification must also be attached.

If these items are not completed and attached, the candidate will be declined.

Covid test is required 72 hours prior to the start date, with results issued 48 hours before the start date.
