About the Role

As a key member of our team, you will conduct a thorough health evaluation during visits lasting up to one hour. This involves reviewing the member's health status, medical history, medication adherence, and social environment to gain insights that are often missed in standard office visits. The Matrix Comprehensive Health Assessment (CHA) will enable you to enhance the quality of care and address multiple care gaps in a single visit. Your key responsibilities will include :

• Performing adult and geriatric assessments, including medical history, diagnosis, treatment, health education, and referrals, while adhering to Matrix protocols.
• Administering and ordering necessary point-of-care and lab-based tests.
• Collaborating with physicians, case managers, social workers, family members, and other healthcare professionals.
• Working closely with Primary Care Physicians for patient education and follow-ups.
• Providing care at the member's residence, whether at home or in Skilled Nursing Facilities.
