Primary Responsibility : The Certified Medication Manager (CMM) is responsible for administering medications in accordance with federal and state regulations and according to the Iowa State Department of Health, under the direction of the delegating Registered Nurse.

Specific Tasks / Duties :

Medication Administration :

  • Administers medication according to policy and procedure.
  • Washes hands prior to preparation and administration of medications.
  • Starts medication pass at the proper time.
  • Keeps the medication cart in sight or locked at all times.
  • Keeps the narcotic drawer locked at all times.
  • Knocks prior to entering the resident's room.
  • Identifies the resident before administering medications.
  • Triple checks the information on the medication label with the Medication Administration Record (MAR) for accuracy.
  • Obtains vital signs (if applicable) prior to administration of medications (pulse / BP).
  • Elevates the head of the bed before medication administration.
  • Administers medication by the correct route.
  • Observes / ensures the resident takes the medication.
  • Washes hands after each resident or uses hand sanitizer with hands washed after resident contact.
  • Does not borrow medications.
  • Completes the medication pass in an appropriate time frame.
  • Crushes medication according to policy and procedure.

Resident Assistance :

  • Assist residents with activities of daily living (bathing, dressing, grooming, toileting) and medication reminders, including identification of physical and cognitive changes, documentation, and communication to appropriate staff.
  • Assist residents with leisure time programming, including escorting them to and from activities.
  • Assist dining services with mealtime serving, clearing, and setting of tables.
  • Assist with resident laundry, including personal washing and bedding change.
  • Perform general housekeeping duties.
  • Perform other duties as assigned.

  • Position Requirements :

  • High school diploma or equivalent.
  • Certified Medication Manager in the state of Iowa.
  • Minimum of one year of healthcare experience or two years of experience as a certified medication manager.
  • Proficiency in electronic health records.
  • Essential Functions :

    Documentation :

  • Medications and vital signs are documented according to policy and procedure.
  • Medications are initialed after administration.
  • Controlled medications are signed out as administered.
  • Omitted and PRN doses are noted / charted / reported to the licensed nurse.
  • Nurse is notified prior to the administration of PRNs.
  • Vital signs are documented prior to the administration of medications (B / P, pulse).
  • Controlled substance count is done at the beginning and end of shift.
  • Discrepancies in controlled substances count are handled according to policy and procedure.
  • Resident Care and Unit Maintenance :

  • Assist with resident care as assigned.
  • Assist with the obtainment of resident weights.
  • Ambulate residents as assigned.
  • Transport residents to activities.
  • Answer all resident call lights promptly.
  • Communicate in a clear, open, positive manner with peers, residents, families, physicians, and support staff.
