California Licensed Practical Nurse

A company is looking for a Licensed Practical Nurse (CA license required).

Key Responsibilities

Conduct outbound telephonic patient outreach to support patients enrolled in care programs

Provide one-on-one coaching and support to patients managing chronic conditions

Document patient interactions and collaborate with the clinical team for care coordination

Required Qualifications

An active compact multi-state LPN / LVN license in the state of residence is required

At least 5 years of clinical experience as a Licensed Practical Nurse is required

Remote work readiness, including stable high-speed WiFi, is required

An active CA state LVN license is required

Fluency in Spanish is preferred
