Position Overview :
Serve as the primary contact for patients and families, guiding them throughout their hospice journey.
Administer and document skilled nursing care in accordance with each patient's tailored Plan of Care, whether at home, in assisted living, or other settings.
Conduct initial and ongoing assessments to track patient conditions and adapt care plans as necessary.
Work collaboratively with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—to develop and refine individualized care plans.
Provide emotional, physical, and spiritual support to both patients and their loved ones, including education, comfort, and guidance during bereavement.
Train caregivers effectively to ensure safe and confident care for their loved ones, offering practical instruction and emotional support.
Communicate essential updates in patient conditions to the care team, facilitating timely interventions.
Maintain thorough clinical documentation and adhere to infection control and compliance standards.
Contribute to quality improvement initiatives to guarantee compassionate and efficient care delivery.
Guide patients and families through vital topics such as medication administration, hospice services, symptom management, and end-of-life expectations.
Qualifications - What You'll Bring :
Current RN license in the state of employment or eligible to obtain.
At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar nursing fields.
A heartfelt commitment to hospice care : compassionate, empathetic, and patient-centered.
Comfortable delivering care in diverse settings, including private homes and facilities.
Strong communication and critical thinking skills in sensitive situations.
Valid driver's license, auto insurance, and reliable transportation.
CPR certification required.
Preferred Experience (Not Required) :
Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
Experience as an Admissions RN, in wound care, float pool, or travel nursing.
Experience working with terminally ill patients or providing serious illness support.
