A social services organization in New York is currently seeking a Case Manager to join their team. This position involves 100% fieldwork across the Bronx and Manhattan boroughs.
About the Opportunity :
- Schedule : Monday to Friday
- Hours : 9am to 5pm (35-hour work week)
Responsibilities :
Providing direct client services, including intake and assessmentManaging a caseload and documenting progress in APSNET and ETOMonitoring risk and adapting service plans as neededReferring clients to appropriate external servicesConducting home visits and telephonic communication weeklyQualifications :
1+ year of Case Management experienceBachelor's Degree in Social Work and / or a related fieldProficiency in Microsoft Office and standard business technologySolid assessment and documentation skillsDesired Skills :
