Position Summary
ChesPenn is seeking a self-motivated, skilled nurse to support the residents and preceptors who provide primary and prenatal care at our Center for Family Health at Eastside. This is an outstanding opportunity to be a part of a dynamic community health practice dedicated to providing the highest standard of care to everyone who needs health services.
The Registered Nurse supports ChesPenn's mission to provide quality health care for all community members by coordinating healthcare services and promoting healthy behavior through patient education and care. The RN's role includes supervision of medical assistant staff.
Qualifications
- RN required; BSN preferred
- Three (3) years in nursing or related field
- Current Pennsylvania Nursing Registration
- Current COVID-19 and flu vaccines required
Knowledge, Skills and Abilities
3+ years' nursing experience, preferably in ambulatory care or community healthUnderstanding of demographics of medically under-served communitiesStrong computer skills, including experience in an EMR environmentStrong interpersonal skills and ability to serve as a supportive member of a health care teamPossession of a valid driver's license and a licensed automobile, preferredAbility to meet approved minimal physical and medical standardGood written and verbal communication skillsWhat You'll Do
Clinical
Provide professional nursing services, including but not limited to; assessment, triage of walk-in and telephone patients, case management, planning of care and provision of care supportive to or restorative of life and well beingManage chronic and acute health problems under protocolCounsel clients on management of their illness while listening to their concerns, fears, and complaintsConduct educational sessions for all patientsAdminister and monitor medications, treatments, injections, immunizations, and PPD testing, as needed, to assist in the provision of carePerform EKGs, blood glucose tests, and other proceduresPerform venipuncturesDress woundsPerform emergency care and routine first-aidProvide on-going patient education, counseling, follow-up, and advocacyAct as liaison for patient in respect to their care with specialists, consultants, other provider offices, laboratories, and insurance companiesRefer clients needing further assessment or case management of episodic and chronic medical, surgical, podiatry, optometry, dental, psychiatric and other health problems identified through assessmentSupervise and support medical assistantsProvide follow-up on identified needs from quality assuranceParticipate in case conferences and other team activitiesTravel to various ChesPenn locations as requestedMaintain confidentiality of clinical records and information in accordance with HIPAA regulations.Maintain a courteous and professional demeanor towards patients, staff members, and all other guests at the health centerClerical
Document all patient contacts, case updates, and referrals in the medical record according to established protocols and standardsCollect and organize patient dataPrepare reports for various organizations requesting information on the patient's medical careConduct chart audits for collection of clinical measures dataAssist with inventories of medical supplies, medications, durable medical equipment, emergency equipment and immunizationsConduct laboratory paperwork as requiredParticipate in meetings and committees to ensure the delivery of quality health careAdministrative
