Utah Licensed Nurse Practitioner

A company is looking for a Nurse Practitioner to provide integrated care for complex kidney patients in a remote setting.

Key Responsibilities

Conduct comprehensive health assessments for patients

Manage complex patients to reduce hospitalizations and improve health outcomes

Work autonomously and collaboratively with healthcare teams to ensure quality care

Required Qualifications and Training

Minimum of two years' experience as a Nurse Practitioner

Current unrestricted NP license to practice in the state

Current CPR or BLS certification

Intermediate computer skills, including proficiency in MS Word, Excel, and Outlook

Willingness to obtain up to six additional state licenses for remote patient management
