GENERAL SUMMARY :

The Substance Abuse Disorder (SUD) / Alcohol & Other Drug Abuse (AODA) Counselor works within an Outpatient Substance Abuse & Mental Health Clinic. They provide group and / or individual mental health and AODA treatment services to clients. They are required to meet all expectations as noted and outlined in DHS Chapters 35, 36 & 75.

ESSENTIAL FUNCTIONS :

• Provides individual outpatient, and / or in-home / telehealth when appropriate and approved.
• Provides group counseling when appropriate and desired.
• Completes assessment, treatment plans and discharge summaries for / with each client.
• Completes progress notes and summary reports to document the direct services provided.
• Submits billing documentation, as required by referral and funding sources, in the agency Electronic Health Record (EHR) system.
• Collaborate with referral sources and other professionals as appropriate (ex : CCS external billing EHR).
• Participate in clinical consultation and monthly team meetings as appropriate.
• Adheres to St. Charles Youth & Family Services Policies and Procedures.
• Attend trainings and meetings as required.
• Other duties as assigned.
