Case Manager - Long Beach, CA | $23-$24/hr | M-F About the Role We're recruiting a
Case Manager to help clients navigate healthcare and social service resources across Long Beach. This role balances direct client support with meticulous documentation and coordination.
What You'll Do- Conduct intakes, create individualized care plans, and monitor progress
- Connect clients with medical, behavioral, housing, and benefits resources
- Coordinate across providers; schedule follow?ups and transportation as needed
- Maintain timely, accurate case notes and reports
- Support client self?advocacy and goal achievement
What You'll Bring- Case management or social services experience (community health preferred)
- Strong communication and cultural humility serving diverse communities
- Organized, detail?oriented, and comfortable with field or phone?based support
- Familiarity with Long Beach/LA County resources is helpful
Compensation & Schedule- Pay: $23-$24 per hour
- Schedule: Monday-Friday
- Location: Long Beach, CA
