About The Position

Job Title : Care Manager

• Graduate of an accredited or state board of nursing approved Registered / Professional Nursing program
• Minimum of 3 years RN Case Management Experience

Description :

Assists inpatients and outpatients and their families in the overall management of their plan of care across the continuum.

This position involves coordinating patient care, advocating for patients, and ensuring that patients receive the necessary resources.
