Job Description & Requirements Optometry Physician StartDate : ASAP Pay Rate : $1455.00 - $1575.00 This facility is seeking a Optometry Physician for locum tenens support as they look to fill a current need. Details and requirements for this opportunity :

• Schedule : Monday - Friday 9 : 00AM - 5 : 00PM with first patient starting at 9 : 00AM and last patient at 4 : 00PM
• Practice Setting : Clinic
• Types of Cases : Routine comprehensive eye exams, pathology appointments, contact lens fittings, contact lens follow-ups.
• Credentialing Timeframe : 60 days
• Electronic Medical Record (EMR) : Point and click
• Certifications Required : Drug Enforcement Administration (DEA)
