We are searching for dedicated Part Time and Per Diem Licensed Practical Nurses with Home Health experience who are eager to become part of our family-oriented work environment. As a Licensed Practical Nurse, you will play a crucial role in delivering direct patient care under the guidance of a Registered Nurse.

Key Responsibilities Include :

• Adhering to the plan of care and ensuring patients receive appropriate treatments.
• Collaborating with the Lighthouse team to achieve positive patient outcomes.
• Documenting care accurately and promptly in line with the plan of care.
• Assessing patient needs and providing essential education to patients and their families.
• Coordinating home health services and communicating patient needs to the Registered Nurse and / or Clinical Supervisor.
• Utilizing equipment and supplies efficiently to ensure quality care.
• Participating in your own professional growth and development.
• Performing additional tasks as directed by the Registered Nurse.
