Horizon Health is seeking two Full-Time Licensed Social Workers in any Horizon Health location or remotely in the state of South Dakota. The Licensed Social Worker acts as a member of the primary care team to help patient with goal setting in attempt to improve health outcomes and will provide counseling and enabling services to patients within clinic setting. Successful candidates for the Licensed Social Worker should be passionate about providing high-quality, patient centered care.
