Hospice RN Case Manager

Responsibilities:

• Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs
• Admissions of patient for services and development of the initial patient/family Plan of Care
• Uses health assessment data to determine nursing diagnosis
• Coordination of services with the patient, family, and hospice interdisciplinary team members

Requirements:

• Valid Registered Nurse (RN) license
• RN Diploma, Associates or B.S. degree in nursing
• Agency Hospice experience preferred
• Excellent observation, problem solving and communication skills
• Compassion and integrity
