Licensed Practical Nurse (LPN)
$27.00–$33.00 / hour – $4,000 Sign-On Bonus Paid out Bi - Weekly
PURPOSE OF YOUR JOB POSITION
Provide direct nursing care to residents and supervise nursing assistants in compliance with regulations, facility policies, and resident-centered care standards to ensure quality outcomes.
QUALIFICATIONS
Graduate of an approved LPN program; current, unencumbered PA LPN license.
Current CPR certification; knowledge of rehabilitative / restorative nursing practices preferred.
Effective communication skills; proficiency with EHR (PointClickCare).
Meets health regulations, background checks, post-offer testing, and continuing education requirements.
RESIDENT RIGHTS
Maintain confidentiality of PHI; provide nursing care in privacy; knock before entering rooms.
Treat residents with dignity; honor refusals and escalate per policy; report grievances / abuse immediately.
Communicate respectfully; recognize cultural differences among residents and staff.
ESSENTIAL RESPONSIBILITIES AND DUTIES
Direct day-to-day functions of nursing assistants; provide leadership and coaching.
Administer medications and treatments per physician orders; maintain accurate MAR / TAR documentation.
Complete assessments and documentation in PCC; participate in admissions, transfers, and discharges.
Notify provider and family of changes in condition; complete incident reports per policy.
Collect specimens; perform accuchecks and vital signs; provide restorative and rehabilitative care.
Collaborate with IDT; participate in care planning and quality improvement activities.
Ensure safe storage / administration of controlled substances; reconcile discrepancies.
SAFETY AND SANITATION
Follow infection prevention, isolation precautions, and hand hygiene standards.
Ensure PPE availability and correct use; safe body mechanics and lift techniques.
Maintain clean and sanitary treatment areas; report hazards and exposures immediately.
Maintain knowledge of SDS; participate in safety education and drills.
SPECIFIC REQUIREMENTS
Move intermittently during shift; lift / push / pull at least 25–50 lbs; assist with evacuations.
Function independently with flexibility, integrity, and sound clinical judgment; supervise CNAs effectively.
