Home Health Care Specialist (San Angelo)

Job Summary

Provides skilled nursing care to patients in the comfort of their own homes.

Key Responsibilities :

Works under the direction of a registered nurse to develop and implement a personalized plan of care for each patient.

Administers medications, monitors vital signs, and provides wound care as needed.

Assists patients with daily living activities such as bathing, dressing, and feeding.

Maintains accurate records of patient progress and communicates effectively with healthcare team members.

Required Skills and Qualifications :

Education :

High School Diploma or equivalent

Experience :

1 year of experience as a licensed vocational nurse (LVN)

Preferred 1 year of experience in home health care

Certification / Licensure :

Licensed Vocational Nurse (LVN) licensure

CPR certification

Valid driver's license
