Home Care Network (HCN) is a nurse-led, locally-operated home health care agency with an excellent reputation as an employer and provider of care throughout Ohio and Indiana since 1993. We are seeking an experienced, reliable Home Health Skilled Nurse (RN preferred) to provide Skilled Nursing services directly to our existing and new clients in Toledo, Ohio and surrounding metro areas.

• This is a long-term employment opportunity for a part-time PDN on night shifts.
• LPN or RN required.
• Current client needs are for 6-20 hours per week during the night, throughout the week.
• 1+ years of experience with Vent Care is required.
• Those seeking full-time hours will have the option to start with part-time hours and first priority to add additional clients / shifts.
• Standard travel for client coverage up to 60 minutes from employee's home is required. Candidates only seeking cases closer to home (
• Mileage reimbursement is provided for travel between clients.
• Home care experience, or equivalent care environments, are required.

Job Summary

A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his / her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN / RN delivers care to clients as delegated by the RN Case Manager.

Reports to : RN Case Manager; Clinical Administrator / Director of Nursing

Weight handled; push / pull force : 10 - 20 lbs. occasionally , 20 - 30 lbs. infrequently ; carry / lift : 5 -10 lbs. occasionally , 10 - 30 lbs. infrequently.
