Nest & Care is currently seeking an experienced Licensed Practical Nurse (LPN) for a PRN position performing home health visits, client assessments, and caregiver training for our clients in and around Montgomery County, MD.
This position is ideal for a Licensed Practical Nurse (LPN) who is passionate about providing exceptional home care to clients. Responsibilities :
Assisting in assessing, planning, implementing, and evaluating patient care;Conducting and documenting ongoing data collection related to patient / family reporting to the RN following guidelines, policies, and procedures;Providing care to the age-specific patient populations such as children, adolescents, adults or older adult-utilizing the nursing process;Providing direct or indirect patient care services including but not limited to medication administration;Demonstrating the ability to perform and respond appropriately in emergency situations;Completing nursing care documentation for assigned patients;Developing and maintaining effective communication with patients, families, physicians, colleagues, and co-workers, to ensure the ongoing provision of patient care;Performing tasks and / or interacting with patients, families, staff, and other personnel in a courteous, honest and compassionate manner.Observe patients, chart and report changes in patients' conditions, such as adverse reactions to medication or treatment, and take any necessary action.Administer prescribed medications or start intravenous fluids, and note times and amounts on patients' charts.Answer patients' calls and determine how to assist them.Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, and respiration.Provide basic patient care and treatments, such as taking temperatures and blood pressure, dressing wounds, treating bedsores, giving enemas, douches, alcohol rubs, and massages, or performing catheterizations.Help patients with bathing, dressing, and personal hygiene, moving in bed, and standing and walking.Work as part of a health care team to assess patient needs, plan and modify care and implement interventions.Record food and fluid intake and output.Evaluate nursing intervention outcomes, conferring with other healthcare team members as necessary.Qualifications :
Graduate of an accredited school of nursing, or proof of successful challenge of LPN Exam.Minimum of one (1) year of recent experience in a home care setting for pediatric, adolescent, and geriatric patients, within the period of two (2) years.Current unrestricted LPN license in the State of MarylandCurrent IV CertificateCurrent CPR and First Aid certificationTB vaccination and Hep B vaccinationGraduation from an accredited and approved nursing program, as indicated by school transcript or diplomaClear criminal background and driving recordTwo professional references within the healthcare fieldPrior home care experience is strongly preferredSelf-starter with the ability to work independently and manage time effectivelyStrong interpersonal skills, including a willingness to teach and mentor caregiversComfort with technology; prior experience with electronic medical records preferredAbility to drive and travel to cases as assignedAvailability for weekdays and some weekends
