Position Summary

The Health and Wellness Director is responsible for leading and managing all aspects of resident health and wellness at Charter Senior Living of Hopkinsville. This role ensures high-quality care, compliance with all state and federal regulations, and a positive, team-driven culture among care staff.

What You’ll Do

Lead, train, and support the care and nursing team to provide exceptional, person-centered care.

Oversee resident assessments, care plans, medication administration, and documentation .

Ensure compliance with state, federal, and company policies and procedures.

Partner with the Executive Director and department heads to promote resident satisfaction and quality of life.

Maintain positive relationships with residents, families, physicians, and outside care providers.

Participate in on-call rotations as needed.

Promote a caring, professional, and fun working environment for your team.

Qualifications

Active RN or LPN license (required, Kentucky or compact state).

Prior experience in assisted living, senior living, long-term care, or healthcare management strongly preferred.

Strong leadership and communication skills.

Ability to manage multiple priorities and maintain attention to detail.
