Job descriptionWork in an acute care facility case management unit Day shift schedule with five 8-hour shifts per week, 8 : 00 AM to 4 : 30 PM Typical patient caseload ratio is 1 : 25 Required to float to the Laguna facility and potentially to other facilities within the state Weekend rotation required Epic EMR system is used and experience is required Responsible for care management and coordination of an assigned patient caseload Scrub color is turquoise or business casual attire required Must complete required onboarding modules pre-start and annually Must commit to working at least 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) No more than 7 days of requested time off will be considered Former permanent staff must wait at least 3 months before returning as a traveler and cannot reside within 50 miles of the facility All travelers must be willing to float between all state facilities as needed Active CA RN license required, pending license accepted if active by start date Minimum 2 years of acute care case management experience performing care coordination required Prior travel RN experience is required; first-time travelers will not be considered Must provide at least two supervisory references covering one year within the last three years and within a comparable setting Ability to perform duties at multiple locations as needed High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
