Registered Dietitian – PACE Program
Pay Range : $42.07–$61.44 / hr
Requirements to Apply
- Bachelor's degree in Nutrition or Dietetics (Master's preferred)
- Active Registered Dietitian (RD) license – Academy of Nutrition and Dietetics
- Commission on Dietetic Registration (CDR) certification required
- Certified Diabetes Educator (CDE) preferred
- BLS certification required (AHA-approved)
- One or more years of experience as a Registered Dietitian (PACE or chronic disease management preferred)
- Bilingual English / Spanish highly desired
Full-time Registered Dietitian opportunity in Murrieta, CA supporting the Program of All-Inclusive Care for the Elderly (PACE). Monday–Friday schedule, 8 : 00am–4 : 30pm — ideal for RDs passionate about nutrition, preventive care, and geriatric health.
Job Overview
We're seeking a compassionate and knowledgeable Registered Dietitian to join our interdisciplinary PACE team dedicated to promoting senior wellness and independence. The RD will provide comprehensive nutrition assessments, create individualized meal and care plans, and lead patient and caregiver education focused on chronic disease management, diabetes care, and therapeutic diets. This role partners with physicians, nurses, therapists, and social workers to help participants maintain health, dignity, and quality of life at home and in their community.
Responsibilities
Conduct nutrition assessments and develop individualized care plans for PACE participantsProvide diabetes education, nutrition therapy, and glucose monitoring guidanceEvaluate lab data and assist with medication management in collaboration with clinical teamsDesign and lead culturally appropriate group and individual education sessions on nutrition and chronic disease preventionServe as a resource to staff on ADA, CPSP, and PACE nutritional guidelinesParticipate in interdisciplinary team meetings and case reviewsDocument all nutritional care and progress in electronic medical recordsMaintain knowledge of current nutrition science and standards of careEnsure compliance with all departmental, safety, and infection control standardsPromote a positive participant experience through courteous, accurate, and timely careBenefits
