NYC Health + Hospitals/Coler, an affiliate of NYU Grossman School of Medicine, is seeking a Board Certified/fellowship trained Palliative care physician to provide outstanding care and ensure exceptional patient outcomes and comprehensive service delivery.

The ideal candidate will:
• Work with a dedicated team of attending providers, Nurse Practitioners, social workers, and ancillary staff to provide holistic, patient-centered care.
• Manage pain and other symptoms, including nausea, fatigue, shortness of breath, and anxiety, for patients with serious illnesses.
• Facilitate conversations with patients and families to understand their goals, preferences, and advance care planning.
• Develop and oversee comprehensive care plans tailored to individual patient needs.
• Collaborate with interdisciplinary teams, including the Skilled Nursing Facility to ensure seamless continuity of care.
• Provide consultation services to other healthcare providers for complex cases requiring palliative expertise.
• Educate and support patients and families throughout the course of illness and at the end of life.
• Participate in quality improvement and clinical innovation initiatives to enhance palliative care delivery.
ESSENTIAL FUNCTIONS:
• Engage with patients, families, and colleagues in line with the organization's ICARE values.
• Maintain detailed and timely documentation in the medical record for all care provided.
• Actively participate in interdisciplinary team meetings and family conferences.
• Deliver education and training to staff on palliative care principles and practices.
• Maintain professional competence through ongoing learning and continuing education.
• Adhere to safety protocols, including infection control and emergency management practices.
Minimum Qualifications:
Active NYS Medical Licensure
Board Certified in Palliative Care
