California Licensed RN Case Manager

A company is looking for a RN Case Manager - Remote- CST or MST Timezones.

Key Responsibilities

Support complex patients by identifying and addressing their medical, social, emotional, and financial needs

Coordinate care transitions and manage ongoing needs in partnership with the healthcare team

Assess patients to identify unreported medical conditions or changes that may lead to adverse outcomes

Required Qualifications, Training, and Education

Current Compact State RN License is required

Minimum of 5 years RN experience required with healthcare performance coaching

Previous Integrated Kidney Care experience required

Current CPR or BLS certification

Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
