Asthma Nurse Educator

MAJOR FUNCTION Under the direction of the Chief Medical Officer, support patients and caregivers to achieve better management of their asthma, COPD, tobacco dependence and / or other respiratory health conditions through education and care coordination.

ESSENTIAL FUNCTIONS   Instructs patients and families on how to manage their condition, including proper inhaler technique, the action of different medications, and how to prevent relapses.    Develops personalized asthma action plans that outline triggers, current treatments, and what to do in an emergency.    Assesses patient needs, monitor progress, and adapt educational strategies as needed.   Helps patients identify and manage environmental or other triggers that can worsen their asthma / COPD.   Works with Clinical and Administrative leadership and staff to maintain a daily schedule of 6-8   Works with providers, clinical team members and frontline staff to coordinate care.   Serves as a resource for patients, their families, and HJAHC staff regarding asthma management.   Provides individualized and small group teaching and skills training for patients.   Works with Clinical and Quality Improvement staff to identify and implement clinical guidelines and workflows to support excellent patient outcomes for patients with chronic respiratory conditions, including but not limited to meeting UDS measures for tobacco cessation and QI measures for asthma management   Greets all patients / clients / customers via telephone or directly in a personal and professional manner.  Promotes positive patient relations.   ADDITIONAL RESPONSIBILITIES :
