Position Details

Under the direction of the Blended Case Management Supervisor, this position will be responsible for developing, coordinating and implementing services for consumers as identified in the service plan. As a member of the BCM team this position will serve as primary contact for youth and young adults. Core responsibilities will include providing case-management, service planning and coordination of services with other agency providers to meet the clients' needs along with supportive counseling, and will include on-call responsibilities.
