Travel Physical Therapist (PT) - $2,618 per week in Wall Township, NJ

Job Title : Physical Therapist Profession : Physical Therapist Specialty : Home Care Duration : 13 weeks Shift : Night Shift Hours per Shift : 8 Experience : Minimum of 1 year License : Registered Physical Therapist Certifications : BLS must be AHA Must-Have : - Minimum of six months PT experience - PC experience / skills Description : Responsible for providing professional rehabilitation therapy to patients in their home. Evaluate, develop, and implement individual goal-oriented therapy programs. Ensure compliance with organization policies and procedures and external organizations. Complete patient evaluation within the specified time frame. Assure the evaluation reflects a complete, holistic assessment of the client's condition. Develop an individualized treatment program based on evaluation findings. Focus on time-oriented, functional, and measurable short- and long-term goals. Prepare appropriate system documentation, including progress notes and discharge summaries. Assure that documentation reflects skilled care and monitors the patient's progress. Submit documentation in a timely manner to support the need for continued skilled care. Ensure service delivery follows the physician's plan of care for the client. Act as a liaison among physicians and other members of the healthcare team. Ensure effective coordination and appropriate resource referral. Act as a supervising therapist to the Physical Therapy Assistant. Assess the PTA's ability to perform selected interventions on an ongoing basis. Educate the patient and / or caregiver in a written home program. Provide comprehensive education based on identified needs in an understandable manner. Develop rapport and mutual respect with the patient to foster an optimum environment. Implement the treatment plan utilizing correct clinical skills and techniques. Independently meet productivity requirements on a consistent basis. Participate in discharge planning throughout the patient’s course of care. Complete scheduling requirements accurately and timely. Respond to voice mail and messages in a timely manner. Document case communication with other healthcare team members appropriately. Assure the discharge summary indicates the patient’s current status and outcomes achieved. Perform other duties and / or projects as assigned. Demonstrate flexibility with job responsibilities in all areas. Adhere to organizational competencies and standards of behavior. Lift a minimum of 50 lbs., push and pull a minimum of 50 lbs., and stand a minimum of 8 hours a day.
