• Monday through Friday\n
• No call schedule\n
• Provider will be taking care of both surgical and non-surgical patients\n
• Have a passion for wanting to help patients lose weight and get their life / health back\n\nResponsibilities : \n\nIn collaboration with a physician, provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.\n
• Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunization and patient care.\n
• Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.\n
• Counsel and educate patients and their families / caregivers concerning preventative health, treatment options and community resources.\n

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.\n
• Nurse Practitioner specializing in Prescriptive Authority credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date required.\n
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.\n\nEducation : \n
