Job Description

The Physical Therapist will provide comprehensive rehabilitative care to patients within a skilled nursing and rehabilitation environment. The role involves evaluating patient conditions, developing personalized treatment plans, and improving mobility and functional outcomes. This is a full-time contract position with a consistent daytime schedule and guaranteed weekly hours.

Responsibilities

• Perform patient evaluations to determine physical therapy needs and plan care accordingly.
• Develop and implement individualized treatment programs focused on mobility, strength, and functional independence.
• Provide direct therapy services in accordance with established standards and regulations.
• Document all patient interactions, treatment plans, and progress notes accurately and timely.
• Collaborate with interdisciplinary care teams including nursing, occupational therapy, and speech therapy.
• Educate patients and family members on exercises, mobility techniques, and safe practices.
• Adjust treatment plans as needed based on patient progress and clinical assessment.
• Maintain compliance with facility policies, state regulations, and therapy best practices.
• Participate in care plan meetings and case discussions as required.
• Provide coverage across all assigned days of the week, including weekends, as needed.
