Key Responsibilities

Conduct comprehensive patient assessments, evaluating medical histories, physical conditions, and functional limitations
Design and implement individualized rehabilitation treatment plans tailored to each patient's specific needs and goals
Perform therapeutic exercises and interventions to improve patient mobility, strength, balance, and functional independence
Provide patient and family education on injury prevention, home safety, proper body mechanics, and recovery techniques
Monitor and document patient progress consistently, adjusting treatment plans to optimize outcomes
Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated care
Maintain accurate, timely, and compliant patient documentation in accordance with regulatory standards
Travel to multiple patient homes within the service area to deliver in-home therapy services

Required Qualifications

Master's degree in Physical Therapy from an accredited program (DPT preferred)

Minimum of 1 year of professional experience as a Physical Therapist in a home health care setting

Valid state Physical Therapy license in good standing

Current CPR / BLS certification (must be maintained throughout employment)

Valid driver's license with reliable transportation and willingness to travel to various patient locations

Strong clinical assessment and critical thinking skills

Excellent communication and interpersonal abilities with patients, families, and healthcare team members

Demonstrated commitment to patient-centered care and quality outcomes
