Job descriptionPerforms thorough patient examinations prior to initiation of treatment. Establishes an individualized plan of care together with the patient with appropriate, measurable short-term and long-term goals and expected functional outcomes in accordance with examination findings. Consults with physicians and other related medical disciplines regularly and timely concerning treatment plans, progression or regression of anticipated goals, and discharge planning. Administers treatment complying with the MN OT Practice Act and facility and departmental Policies and Procedures. Monitors and assesses progress or lack of, response to treatment, and results of treatment on a continual basis. Assures continuity of optimal quality of occupational therapy care to patients. Continues OT justified either by evidence of problems that necessitate treatment or by evidence of observed or expected improvement in functional ability. Provides home care education and instruction and assists in discharge planning. Evaluations are thorough, complete and comprehensive. Therapeutic exercise treatments are administered according to accepted standards paying particular attention to indication and contraindications for specific medical diagnosis and considering patient tolerance and response. Functional mobility and activities of daily living treatments are administered according to accepted standards paying particular attention to indications and contraindications for specific medical diagnosis and considering patient tolerance and response. Involves patients and family in treatment planning, home programs and discharge planning as appropriate. Fabricates and orders various splints, supports, braces and durable medical equipment as appropriate with thorough patient and family instructions as necessary. Provides therapist contract services as assigned. Attends patient / resident conferences and meetings as assigned. Demonstrates a respectful, enthusiastic, motivating attitude and approach towards patients and staff while showing solution-oriented involvement at all times. Carries out directives in a positive manner showing self-initiative and accepts responsibility for follow-up. Is challenged by change, recognizes need for change, views constructive criticism a tool for improving self as well as patient care. Demonstrate strong work ethic with effective time management skills. Demonstrates good communication skills with all staff and directs concerns to the appropriate individual(s), avoiding gossip. Promotes MLHS throughout the facility and community. Attends and participates in all mandatory and voluntary facility and departmental meetings, education and training sessions, groups and committees. Values and improves current level of expertise by participating in continuing education. Assists in in-service education and community education programs as directed. Completes initial evaluations, plan of cares, recertifications, progress summaries, and discharge summaries on a timely basis following policy guidelines. Completes requirements for durable medical equipment. Completes patient billing requirements timely. Assists in maintaining statistical data. Completes all other contract service or special program required documentation as specified in the corresponding policies and procedures. Responsible for safeguarding any PHI seen, used or disclosed during this positions normal job functions and stated in the above job responsibilities based on the HIPAA privacy and security policies and procedures. Familiarizes self with, supports, and adheres to all departmental and facility safety policies and procedures reporting violations immediately. Satisfactory completion of the academic and clinical requirements of an Accredited Program in Occupational Therapy. Current license to practice occupational therapy in the state of Minnesota. Maintains mandatory continuing education requirements. Demonstrates confidence in occupational therapy knowledge base and experience. Ability to work in a team setting and establish good rapport with all co-workers. Adaptability and flexibility to changing work situations and schedules. Excellent verbal and written communication skills. Possession of attention to detail, organization and prioritization skills. Requires a valid Minnesota driver's license and insurability with the facility carrier. Maintains a good record of attendance and punctuality. Summary
Conducts medically prescribed Occupational Therapy evaluations and treatments with Mille Lacs Health System's mission and vision. Practices with the patient as the focus striving for achievement of optimal quality and customer service of care at all times. Evaluates and treats disabilities, injuries, and diseases of all age groups. Includes facilitations, cooperation and collaboration with other departments, medical staff, and healthcare facilities.

Position Type / Expected Hours of Work
Professional position. Hours of operation are Monday through Friday, 8 : 00 am to 5 : 00 pm. Involves holiday rotation. May involve work in a patient's home, off-site contracts, and may involve driving.
Supervisory Responsibility
Includes supervision of occupational therapy assistant, departmental support staff and students.
