Sunbelt Staffing is hiring an experienced Home Health Registered Nurse (RN) for a 13-week contract assignment in Lavonia, GA. In this role, youll deliver compassionate, individualized care to patients in their homes while supporting a well-respected national home health agency.

Contract Details :

  • Location : Lavonia, GA , and surrounding areas
  • Company : Sunbelt Staffing
  • Contract Length : 13 Weeks (with possible extension upon successful start and completion)
  • Hours : Full-time, 40 hours per week, no guaranteed hours
  • Schedule : Monday-Friday, 8 am-5 pm, with occasional weekends and on-call duty (typically one weekend per month on-call)
  • Compensation : Negotiable, based on experience and territory budget
  • Mileage Reimbursement : .48c from patient home to home, 150-250 miles per week expected travel
  • Blended Pay Packages : Available for travelers
  • Benefits : Medical, dental, vision, 401 (k) match after 90 days
  • Productivity : 30-32 per week, up to 35 patients per week

Job Responsibilities

  • Conduct comprehensive patient assessments using OASIS, ensuring accurate documentation and compliance with agency standards.
  • Develop and implement individualized care plans, coordinating with physicians, physical therapists, and other healthcare professionals.
  • Administer medications, monitor vital signs, and provide skilled nursing care, including wound care, IV therapy, and catheter care.
  • Educate patients and their families about care plans, medication management, and other health-related topics.
  • Maintain accurate patient records in Homecare Homebase (HCHB), ensuring compliance with regulatory requirements.
  • Travel to patient homes within the area, using reliable transportation.
  • Participate in interdisciplinary team meetings and case conferences to ensure coordinated and effective patient care.
  • Adhere to Joint Commission standards and other regulatory requirements.

    • Requirements

  • Active Registered Nurse (RN) license in the state of Georgia.
  • Minimum of 1 year of home health experience.
  • Proficiency in OASIS and Homecare Homebase (HCHB) required.
  • Oasis Certified strongly preferred.
  • Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.
  • Reliable transportation with a valid driver's license.
  • Ability to meet Joint Commission onboarding standards upon offer.
